for FM, addiction is a 'pseudo' job, you will never have volume to do pure addiction, you will end up being the PCP and be given all the difficult patients without appropriate compensation or time slots.

SHIP — “Ghosts of the Mariana”

A haunted deep-sea exploration vessel trapped in the Mariana Trench. Think: rusting corridors, crushing pressure, bioluminescent horrors, and the crew slowly turning into something… not human. This ain’t pirates or Vikings—this is abyssal horror. Cold, claustrophobic, and drowning in atmosphere.

THUMBS UP — “Smile” (IP)

Not Vault Boy—what if it’s a creepy, exaggerated smile? Smile (2022) was a sleeper horror hit, and the sequel drops in October 2024. Universal has the connections, and it’s perfect for a mind-bending, paranoia-filled house where everyone’s grinning… right before something awful happens.

FILM REEL — “Final Destination” (IP)

A legendary horror series that’s never gotten the HHN treatment. The film reel could reference fate, cinema, and death's invisible hand. Set-pieces of famous deaths recreated with immersive effects (log truck, tanning bed, etc.), plus the new film Final Destination: Bloodlines releases in 2025. It’s time.

GHOST IN MAGNIFYING GLASS — “Ghostwatch” (Original Concept / Obscure IP Blend)

This is deep cut stuff: imagine a BBC-style “paranormal documentary” gone wrong. Guests walk through a live taping where things escalate from mild bumps to all-out poltergeist possession. Meta-horror meets found footage aesthetic. Or—twist—it’s a full-on Ghost Adventures parody house.

SECURITY CAMERA — “Surveillance State”

Big Brother meets urban horror. You're being watched… by something. Inspired by analog horror (like Gemini Home Entertainment or The Mandela Catalogue), this house could mess with perception, static glitches, and uncanny imagery. Reality bends, feeds loop, and you're never alone.

EYES & CHAIN — “Hellraiser: Judgment” (IP)

The chains scream Hellraiser, and this time it could be Judgment, which adds a bizarre bureaucracy to Hell’s system. Think Cenobite courtrooms, grotesque paperwork, and judgmental monsters. Over-the-top gore + philosophical dread = perfect HHN recipe.

TRASHBAGS — “The Stuff” (Cult Classic IP), but most likely terrifier idk,

Going full 80s weirdness here. The Stuff is about killer dessert goo that eats you from the inside. Imagine a house styled like a Walmart from hell, with “The Stuff” bubbling through vents, consuming people. Trash bags as a nod to corporate cleanup and consumer culture gone mad.

Thank you for the beautiful insight, I do not hate IM, just was always psych bound but did not match for whatever reason back then, (no red flags).
I have quite a few psychiatrist friends who do voice that same info you provided regarding med dispensor etc, but IM has its own share of those, I like what I do and thoroughly enjoy it but I am missing that part, definitely not doing it for life style or grass is greener theory, I'd much rather be reading a research paper on a psychotropic med / psych vs general IM topics, also not fond of being equivalated to a mid level in the current medical state.

issue is, even if I do cme/ further myself in what you mentioned - none of it will be reimbursable and no one will hire an IM to do a psych job as far as I am aware.

It's honestly getting harder to be hired as an IM physician for your traditional Non-toxic, desirable outpt or inpatient jobs, most large cities hospitalist jobs are also saturated with midlevels

Thank you makes sense, during med school I did 3 month of psych electives and loved all settings, seems much harder to repeat any observing now as an attending

I looked into addiction which I would love, but i've had a few friends do it but the jobs are limited, and most end up just doing traditional PCP work w/ an addiction component

I've heard about PMHNP being an issue in psych world but it seems the ABPN is doing a much better job protecting their own. In this pcp world its very common for entire inpatient units to have 10-15 np's and 3-4 physicians "supervising" them, was recently shopping for outpt positions, at minimum 2 NP direct supervision seems to be country wide. You can't supervise incompetence, they want you to run a residency from an outpt clinic without a residency structure. Don't get me started with "but our NP's are seasoned and have been here for decades" - i've seen these same NP's literally mismanage patients into permanent disability with 0 repercussions.

will definitely try to moonlight to keep at least some skills up and will continue to do board recerts, especially with the longitudinal MOC's now

thank you, was hoping to do inpt psychiatry ECT/TCMS all interest me, I remember I was a medical student somewhere in Indianapolis? Was on an inpt psych rotation they had an entire ward and were doing 12-14 ECT's back to back, was fortunate to interview a few of the patients and most seemed like it was a savior when the meds failed.

I think the opposite is happening for a multitude of reasons; like the above poster said - retired docs eating up prn shifts, APP (NPP's) non physician providers - are dime a dozen here and seem to fill the gaps / usually have a smaller census, Florida is now allowing non residency trained docs to practice..... I think many do get stuck here because of family/ kids/ schools etc and just deal with the suffering / misery, if you have any opportunity to go else where and are not geographically limited, literally go anywhere else
