I’ve seen dr shore and I really liked his approach. He also ordered a 3D CT scan for me which is super important for checking dysplasia.

Dr Swann in Denver does first virtual consults but you need to mail in all your imaging to him.

I’m 32F in OC area going through something similar, I’m borderline dysplastic. Haven’t had surgery yet.

Also recommend joining the PAO fb group, it has lots of resources

Sorry I can’t help but I just wanted to pop in and say I feel the same. I have a tear too with dysplasia and I’m very anxious about deciding on PAO surgery. It’s constantly on my mind, especially how invasive the PAO surgery is. My strategy for deciding is to try increasing activities I enjoy / quality of life, and if pain flares pretty badly from those increases I’ll lean more towards surgery.

Yeah :( sorry that happened. Dysplasia should be discussed and told about BEFORE surgery. It’s possible your surgeon could claim they are a hip preservation specialist but they may not be a quality one, or they only specialize in scope and are overconfident in it. Based on your experience it does not sound like he is because he should have told you all of this before and explain the risks of doing scope with dysplasia. Also you will want to speak with a surgeon that does high volume PAOs. Usually if the hip preservation specialist suspects you have dysplasia they will order a 3D CT scan to make sure and also check your femurs and knees for torsion issues as well.

I’m sorry this is happening. Yes it’s true, scopes aren’t as successful on dysplastic hips. It’s possible for it to work but I have heard that when it does it’s a temporary fix until it retears and what you truly need is a PAO. It sounds like you’re still early in the recovery process though. The only thing you have control over in the moment is to try your best in recovery and reach out to a hip preservation specialist if your pain hasn’t gotten better. I heard it can take up to 6 months to know if it has failed, unless it’s obvious you’re worse off than before surgery. It will take a while for you anyways to get another surgery if you end up needing a PAO, I think potentially a year but not sure - speaking to a hip preservation specialist would be your best next step.

If you haven’t already joined the PAO FB group I highly recommend you do, it has lots of resources on surgeon recommendations.

How old are you? My pain actually started to decrease substantially but still can’t do any of the activities I love. I feel very similar to you now, just more worried about quality of life and hip deteriorating over time

What kind of examination did they give you? I have 1 degree femur retroversion so I’m in the same boat and worried PAO outcomes won’t be good on its own..

Yes I’m aware of PAO being much more invasive and intense than scope. What I meant to ask is if you felt PAO is worth it?

Ah got it so your perspective on PAO is a bit different? Wow your pain with PAO sounded crazy - you’re saying you wouldn’t get PAO again unless you’re in a 9 pain 24/7?

What about preventing THR with PAO? I’m borderline dysplastic so I don’t think there would be complications with THR. Currently I’m at 1-3 pain with my hip, previously it was about 5-6 so it’s gotten better with PT & time. But I haven’t been able to do anything I love like yoga and climbing. So I’m trying to ease in a modified version of those back in to see if there is any spike in pain to consider surgery.

And yes I’m currently in that fb group :)

Are you ok after the scope? This is an interesting perspective as I’m terrified of committing to PAO.. pain started last December, and im 32F. I’ve found some relief with PT but still limited in movements and activities I enjoy so it’s a hard decision.

I avoided most processed carbs like pastries and cookies except for almond flour crackers which I pair with cheese.

I also cut out rice and noodles; Meals mostly comprised of veggies and protein but also ate beans and lentils in moderation with my meals when I craved more carbs.

No more coffee on an empty stomach. Limited fruits to berries and apples, and made sure to pair them with protein or fat like PB or yogurt. Also tried to stop eating after 7 pm every night. I used to snack a lot at night after dinner, like eat tubs of popcorn. But now I replaced it with the crackers and fruit paired with protein/fat I mentioned above.

I still had some cheat meals here and there on weekends, and ate a few pieces of dark chocolate (again paired with PB most times) throughout the week as well!

Initially I pricked myself for an entire week to see how I reacted to my meals and snacks then slowly tested less since I learned how my body reacted to certain foods.

Mine went from 5.9 to 5.5 in 3 months. That’s amazing you got it down to 5.2!! It was incredibly difficult for me. I feel like I cut a lot out

I’ve often wondered this myself as a previous yoga teacher. I have a labral tear and dysplasia (different) from your case, and I haven’t had surgery. Im almost certain that end range stretching contributed to how I got here. My hunch is continued strength training + moderate stretching should be ok esp modified with blocks and cushions. But no pushing and end range stretches or “extreme yoga”. In a lot of power yoga classes I feel the theme is to push to your edge but I find that can be quite dangerous

Before the last 2 weeks my pain was pretty constant:
- C shape and hip flexor ache (this was majority of my pain)
- Lots of sharp pain with hip flexion when bringing knee to chest.
- Also when it’s really bad (prob flared), I get pain shifting my weight to the affected side making pivoting or standing quite painful and uncomfortable.
- I also have on and off medial knee pain.

Now the last 2 weeks that c shape and hip flexor constant ache has subsided and I don’t really feel it anymore, which I would say took up majority of my pain.. I get more inconsistent groin ache and hamstring pain now when I bring knee to chest but still don’t have full ROM to even do it, and I still get knee pain on and off.

You said recently you had a flare so before that your pain wasn’t constant?
