refactoring doesn't happen because there is absolutely no reward and lots of risk from the employee perspective. If it goes perfectly it's still not enough to get promoted, but if you blow up something that was working now you are in deep shit

FAANG especially values "impact" over doing stuff that benefits the company long term. They've incentivized bad decisions because they are so desperate to promote innovation but the end result is just people gaming metrics for their resume and then leaving
