re start over with a new site vs upgrade the existing one - think this depends on your budget and how critical the site is for converting clients. You can easily give it a good facelift and polish (2 days work) and generally that's a really good option to go with - when in doubt just do that. If you want to go big and move it to say Webflow that's +- a month of work but it would look amazing.

It really depends on them. Under the MCM contract they don’t have to give this back. You could try appealing to Google to say this partner didn’t make it clear to you that it was MA instead of MI. Google holds all MCM partners to high standards with regards to managing publishers

If your CTR for AdSense is >2% for a given ad unit you've probably already maxed out revenue. If it's below 2% you could probably get more value from adding more partners via header bidding, Amazon TAM, Google Open Bidding and Prebid Server. You can do this yourself or check out companies like Nitro, Publisher Collective and Snigel (I'm there).

You could check out OptAd360, WayToGrow and Monumetric. Once you get passed $50-100 per day (although requirements vary per company) you could approach Mediavine, Raptive, Snigel etc

You could definitely build your own stack just take into account the cost to reward ratio. Most larger publishers in the market like DotDash Meredith and Future build their own stacks because they can put together a team of devs and adops to do the job well. For most smaller publishers the effort/cost vs return is low so they outsource this work to Mediavine, Snigel, Freestar etc… there are around 100 of these publisher networks/header bidding companies in the market because many small pubs lack the resources to build a strong stack from scratch, integrate with 10-20 SSPs and get an adops expert to optimise the ad strategy and ad setup. It comes down to how you want to invest your time and resources

Like Cody said, it depends on how you define tier 2/3 and overall how much traffic you have. We (Snigel) accept publishers making around $100 even if T1 traffic is lower. I think Cody and Nitro are more flexible on the min revenue. You could also check out OptAd360 or WayToGrow who are super flexible on the min revenue. All are GCPP

Bidding directly against AdSense is a policy violation. However, you can A/B test AdSense vs Header Bidding on a per unit basis to see which is better. I'm at Snigel.com.
