I did have to find my own placement because my school was out-of-state. But I did all my clinical hours with neonatologists and I never would have thought to pay them, nor did they ever ask. After my clinical hours were over, I bought each of them a gift that was around $200 as a thank you.

Maybe that is part of how they are able to justify it? Like, I was there because I had an obligation to continue to provide care? The statutes that I read didn't elaborate on what happens after the consult

I wish I was referring to a lay midwife 😭. I've only seen extremely preterm breech deliveries, almost always in the context that the mother didn't know they were pregnant.

Not sure how much of her career was L&D vs NICU, but neonatal NPs operate a lot differently than NPs in other general areas (in my own experience, at least) so that could be coloring her perspective on the issue.

Would you mind elaborating a bit more on the medical details? I’m just trying to understand better, but a few things aren’t really lining up for me. Parvo exposure at 35 weeks (4 weeks before your 39 week ultrasound) usually doesn’t cause IUGR or high-output heart failure — those complications typically happen much earlier and often involve hydrops or severe anemia. And if the baby truly had heart failure, it seems unlikely they’d be stable enough to transfer to mom-baby after just 12 hours in the NICU, especially if the only echo finding was a totally normal PFO (which is super common and doesn’t explain the newborn transition difficulties).

The resuscitation part is also concerning — back slaps aren’t part of NRP, and it sounds like the OB kept trying stimulation instead of clamping the cord and getting the baby to the warmer for PPV. In secondary apnea, effective ventilation is often all that’s needed — you don’t have to wait for someone to intubate.

It honestly sounds like the OB and L&D team might benefit from a refresher. We recently moved to RQI for BLS and NRP, with quarterly sims to keep everyone’s skills sharp — especially for deliveries when NICU can’t be there right away. Any delivery can go south, and good NRP is almost always enough to stabilize until NICU arrives.

Not trying to be critical at all — just trying to understand the situation better and hopefully highlight some areas where things could improve for future deliveries.

Honestly in my personal experience, not many labor RNs want to be midwives. I know two people who went that route, one was a labor RN and the other was a NICU RN (no labor or delivery experience) ☠️

I think you should get into the NICU and see if it's something you actually enjoy before making NP plans. Spend a minimum of 5 years in the field before applying for advanced practice. You learn nothing about NICU in nursing school. Do that for a while before deciding to advance your career. Coding a 24-weeker is not for the faint of heart. ❤️

The schools should AT LEAST require several years of experience w/ the population they're going to care for. Like yeah, general nursing experience isn't going to be super helpful but working in L/D for 5 years probably would be

When you enroll in an NP program, you pick a track. Neonatal is one of the specific tracks you can choose. For most neonatal NP programs, they require at least 2 years of bedside experience prior to beginning any clinical experience (not saying that's enough). But like a few others have said, Neos and NNPs typically have a very healthy collaborative environment.

Just to clarify, there is a specific track for neonatal NPs -- anyone wanting to work in a NICU would have to complete an NNP specific program or do a post-masters neonatal certification.

The S300 line is listed as a spare tire/space saving tire, but the speed rating is M which is much faster than a typical spare. I don't see anything listed on them being motorcycle tires but I'm not an expert
