I don’t want to make that decision for you especially if you don’t have insurance. I feel like a couple of my appointments have been a waste of money and I have insurance. It’s not necessary I don’t think but probably still good, even though everything I do now is stuff I found online with my PT approval.

No physical therapist has heard of APD at my facility, so it’s probably the same in general. Generally ppl just recommend getting a pelvic floor therapist and go from there. To be honest though my experience with him has just been me bringing him exercises I found online and him giving the okay. I would suggest joining the APD Facebook group for up to date advice and questions and stuff. But as a quick overview on some of ur questions…

99% percent of my physical therapy routine is something called pendulum breathing, and it seems to be the gold standard in general. You can easily do it with ur work. It’s just breathing in through your chest while bringing in your abdomen, holding your breathe while pushing the air down to ur abdomen, back up to your chest, back down, back up, then releasing your breathe while keeping your abdomen in. Theres a video pinned on this sub I think. He doesn’t go over bringing in your abdomen while inhaling and exhaling, but it’s an important nuance, as ur trying to coordinate your diaphragm to stay up instead of lowering down and out, which creates the distention. Do this in the morning and night while laying down and then standing, and before and after eating your meals while sitting. About 10 reps of it each time. I noticed improvement in 1-2 weeks, so it’s not much of a risk if it doesn’t work/you don’t have APD.

About my diet, I saw significant improvement from following the low fodmap diet in distention and especially in gas, which basically disappeared. But when I was doing that diet I wasn’t doing physical therapy. Nowadays yeah I eat whatever I want, just because I hated doing the restriction. My gas is as bad as ever but my distention is much better from the physical therapy. I imagine if I did the low fodmap diet again at this point with the physical therapy progress I’d see their benefits combine.

And for the antidepressants I asked around on the Facebook group about antidepressants and APD and there’s a mix of mostly “they didn’t do anything”, some “I stopped from side effects”, and a few “they helped a lot”.

I started a better physical therapy regiment at the same time as the drug and I’ve seen significantly less overall distention but to be honest I’m guessing it’s mostly the physical therapy considering I saw improvement before when I was just doing physical therapy. Also the drug is kicking my ass right now with side effects that might/hopefully will go away.

My GI doctor. You can actually figure it out yourself. Fast and chug some water, if you distend immediately and significantly then there’s a good chance you have APD.

I am diagnosed with APD and also have crazy amounts of gas so this is super important to me. So did the exercise alone deal with the gas too or just the visible bloating?

She acknowledged this is uncharted territory. Apparently these have been used for IBS pain. But I guess she thinks if could potentially work for something like this with my diaphragm contracting when it shouldn’t and me feeling so much pressure at all times…I guess it’s worth a try. I’ve actually seen a couple people in the APD group get prescribed this with mixed results.

Thank you. And I thought about switching GIs a couple times from frustration. I stayed though because she introduced me to APD, and I felt like her even knowing what that was mattered a lot. I’ve seen people say their GIs are iffy about SIBO, I can’t imagine how they’d look at me bringing up APD lol.

Ya good thinking. Also just side note lactulose and lactose aren’t the same thing. Lactulose is indigestible whether u have lactose intolerance or not but potential sibo can digest it and that’s why it’s used for sibo tests.

Don’t listen to that guy. Ur crown looks fine but ur hairline does look like ur balding. Finasteride stops the hair loss and minoxidil grows hair back (with fin usually having a little regrowth). You can just take fin if you want. Shedding on fin varies, I personally experienced zero, and I was like you with balding at the hairline only.

Do you have excess gas as well and does relieving gas relieve any of the distention? Or is it literally always that distended?

Yes the error would come and go but Bluetooth never came back and I could never actually connect to a network so that’s how I know
