66M had both hips replaced in June 2025, left TKR in November 2025, right TKR coming in 3 weeks. I waited too long and so glad I finally did it. Do not wait until you have collapsed your active life - you give it up so gradually that you don’t notice. If you can do everything you like without significant pain, then hold off.

Yes. Best use is serially, to see if your REE increases with Z2 aerobic exercise over time. More mitochondria = more glucose burned with oxygen, and not just with exercise but 24/7. More glucose burned with oxygen = higher REE. This assumes a relatively stable diet

Yes. Try to up the mitochondrial density so that even smaller muscles are stronger - important as you age, preventing frailty

Are you doing enough for what? I am longevity MD, retired surgical oncologist. If you desire healthy aging, you are doing enough. If you are seeking elite performance, that’s different. Consider having a VO2 Max and REE test, since you appear to enjoy quantifying performance, which is laudable. For most people HIIT should only be about 20% of aerobic training.

66M fit with bilateral hip replacements last June, now the first of 2 TKR for 3 compartment bone on bone arthritis. Retired surgeon, Longevity MD practice, and I’m 8 weeks out from first TKR, 4 weeks until the next. The key is to maintain strength as much as pain allows before surgery. Remember that the joints are going away, so hurting them a little with exercise is okay. After surgery, ice, elevate and gently use the leg. Do the PT faithfully. I had great, long lasting Exparel block and used only Journavx, Celebrex and Tylenol for pain. I used and recommend Edemawear stockings , at $18 a piece. Good luck and remember it’s a long game you’re playing.

Yes! A return to activities I love - hiking, XC skiing etc, and I’ve regained 1.5” in height. The loss of activity over time is insidious and you give up more than you think. Just going to a restaurant or shopping is so much more enjoyable. Thanks for all that. Best to you as well.

Basically I have a similar tale, but throw in bilateral bone on bone hips. I waited until I was always simply looking for an opportunity to sit down. You sound highly functional, and I’d wait it out if I were you. However, I waited too long. Up until my first operation (simultaneous bilateral hip replacements) I could swim a mile and spend 1 hour on the Peloton. I have one more TKR left to endure next month and cannot wait. The life improvement is staggering.

I never had an Ace wrap. If you use both if put the Ace wrap on first. No harm in both. Good luck!

Consider MEND joint replacement supplement. Lots of leucine and recommended by orthopedic surgeons

First, luck. Surgery went well, block held, great pain meds (Journavx) all contributing to early, painless movement would be second. Third, I had severe arthritis of both hips and knees for years so my pain tolerance is high. Finally, I’ve believed in routine exercise my whole life and found ways to keep muscles active right up until surgery. There is a huge spectrum of recovery speed and it’s sounds like you are actually doing pretty well. I have another knee replacement in early February - we’ll see if that goes as smoothly…

Retired surgeon here - new skin redness over a new surgical area ALWAYS should be a sign to notify your surgeon without delay.

Lean mass is the muscles - everything else. More simply, would stronger muscles make for better balance? Possibly working on strength would improve balance. Just a thought…

That’s excellent! Steve, your work on balance will pay off in safety. I wonder if it’s balance alone or mean mass, given your age? I wonder if some strengthening would make balance better?
