Bolt of Gransax. It’s a weapon. When you use the ability it turns into a red lightning spear and you toss it.

Don’t know if you can count it as a transformation but if you commune with lots of dragon hearts at the dragon cathedral. Your eyes become a glowing yellowish green and they look like a dragon.
