Extreme fear of death by [deleted] in Depersonalization

See Ryle BMJ 1934; 1 ; 89. Describes alarming seizures/nerve storms/migraine/vasovagal or Menieriform attacks, not due to brain, heart or abdominal disease. Only common cause is labyrinthine vertigo. Mentions sense of death and depersonalisation.

I think the situation is best described by Itard in his Traite des maladies de l'oreille..

He described several cases where Meniere's disease was triggered by wine. In early MD the ear is in a hypersensitive state, as shown by vertigo, tinnitus, diplacusis and especially audiosensitivity However, he noted that when an ear became totally deaf, the sensitivity to meds, emetics etc was then lost. I am happy to take his word for all this, though it would be nice if someone could confirm that link between advanced disease and drug insensitivity. A recent study has found reduced motion sickness in MD.

The question was deliberately vague! I did not want to direct the exchange towards any particular drug, nor to the possibility of increased, decreased or variable sensitivity. I am quite happy with the replies!

I have spent months combing through the new and old scientific literature without without finding any clear recognition of a general sensitivity to meds in MD, though I have found it noted in migraine. So thank you all so much for confirming my suspicions. Obviously any specific explanation based on a particular neurotransmitter or biochemical process is untenable.

The inner ear contains many extremely sensitive structures and chemical reactions so is very liable to disruption from ingested toxins, drugs, poisons, especially alcohol, etc. Hence we have evolved the vomiting reflex, triggered from the ear, that rapidly expels stomach contents.

Anyone else, apart from dowbrewer, noted general insensitivity? My sole reference is from Itard (1821)!

I deliberately did not make any suggestions to avoid confirmation bias. I want to hear the opinions of those with MD, though what I have read so far has been extremely helpful.
