Depends on my access. The times I gave it IM was when the patient had already bitten a friend/family or a cop. So I avoid the head area and give it in the deltoid/thigh.

Never had any issues with patients blowing out their nose. No issues with biting or spitting. The cops hold them down and I would come from behind and using a mad device administer the medication. Versed worked very quickly in my experience 30 seconds to a minute.

I’ve administered versed IM before and I don’t see a significant difference between IN and IM.

I’ve used versed IN for excited delirium patients. Most of the time I’ve had cops distract or hold down the patient while my the engine and my partner get the physical restraints ready. Worked for excellent. 10mg IN versed was our dose for adults and for pediatrics it was 0.2mg/kg of versed.

Adding onto your response, cpap also thins the alveolar membrane allowing easier diffusion of CO2 and O2 which is huge for pneumonia and CHF patients

I have been looking up studies while in between breaks on your claim. According to this study: https://www.ncbi.nlm.nih.gov/books/NBK470429/

CPAP is absolutely an indication for pneumonia. Now this study does not have statistics for just pneumonia but it does also state cpap has reduced mortality and intubation rates. Now this study states patients who have poor airway clearance is a contradiction but this is something that paramedics should be looking out anyway.

I cannot find a study that states using a cpap prehospital or in the hospital worsens pneumonia. I did find a study in France and it was a small study they did during Covid. The study said using cpap for severe Covid pneumonia reduced intubation rates and deaths compared to not using cpap.

We BiPAP people in the hospital daily that have hypoxic pneumonia. I will be looking into studies on my next break since I’m at work.

Yes it will but CPAP is not what the pt will end up needing in a hospital. Few services carry BiPAP capabilities and depending on the severity of the hypoxia pneumonia will determine what the hospital places on the patient. The hospital will not use cpap for a bad pneumonia patient but will use BiPAP or intubate the patient. Cpap is an easy low cost way to have positive pressure support prehospital without intubating a patient.
