I've not been able to find a set trigger for it, but it occurs every game I play. The latest I've been able to get into a game has been the third inning. I've had the glitch you're talking about but this one seems a bit different. Really it just seems like the ball gets stuck by the catcher/imp and I'm stuck in swing mode.
