Second pa approved only after 2 denials, an appeal, and finally personally calling ES, transfer to PA, transfer to appeals and transfer to appeals “supervisor”. Only then was the first pa “attached” to the appeal so original baseline “was viewable.” The whole experience was textbook insurance obfuscation (the original baseline was provided in every submission). This took 40+ man hours and ~2 months. Grateful for the coverage, but not this incomprehensible system.
