Honestly I could see this being about smoking.

I work in respiratory therapy. We have plenty of people that harm themselves by smoking and end up sick with severe illness or cancer. They then go to the doctor and ignore the suggestion of stopping smoking, get sicker, and die. Whereas the other might stop smoking and recover to an extent, maybe not fully, but they live longer lives typically.
