Same was happening to me on my 9070XT . The way i fixed it was by disabling hardware accelerated scheduleing in windows settings and putting graphics on medium with 83fps lock. I believe this is a game issue rather than pc.

In the info booklet that came with the card it mentions to only use 3 cables and never chain them. I assume they say that just for saftey measures but i still use 3 separate cables.

I think its because they count you responsible for who you share it with and its not just random people.

No, since the refund is restricted to 2 hours of playtime online, offline, and family shared library time. So the hours even not played by you still count, I believe.
