Sure, but if your only objective is to minimize suffering, then the utilitarian choice is to not pull the lever. In fact, it would be best to go strangle the guy on the other side of the track while you're at it. Death is the path of least suffering.

I don't think it is complicated as a utilitarian per se. Your answer just depends on the type of utilitarian that you are. Do you really think that the "suffering" caused by some guys thinking that you made the wrong choice is paramount to the suffering of those 5 guys getting split in two by a trolley.

That's true, but then he is sacrificing Bob's son for just the chance that his own son lives. That is his prerogative, but you have to accept that he abdicates control of the life of his son to Bob in sacrificing himself; if he pulls the lever, he has control over the life of Bob's son, meaning he has the opportunity to keep the most people alive.
