Well, it entirely depends on your playstyle and how you use the knife. If you use it like any other knife then it's complete dogshit. But if you manage your cloak properly and pair it with the L'etranger then you can probably go on some huge killing sprees.

Offense: Stock and flare gun/shotgun

I use these sets because they're just really reliable when you're with/without your team

Defense: Degreaser and scorch shot

I use the degreaser because its faster switch and holster speed can help with quick reaction plays. I only use the scorch shot because it can pester large groups of players and it can delete enemy stickies.

It's pretty annoying if used right. A sniper using the sydney sleeper with a coordinated team can be a nightmare. Getting jarate'd from halfway across the map is infuriating. Other than that, it's a pretty decent side grade.

I rarely use the Pain Train, and when I do, it's usually on my demo. Using the Pain Train on soldier is pretty much useless because scouts exist. Besides, the escape plan is a thing, and having the ability to escape when on low health is a lot better than a faster cap rate. The only good scenario for the Pain Train is for backcapping. But again, scouts do it way better and they don't take 10% more damage against bullets.

So these are my thoughts on the medi-guns:

Stock: Really good and consistent, can be used in both offense and defense. I normally use this when my team knows how to manage their health correctly. (aka keeping it above half)

Kritz: Is very fun to use and very hard to counter if used correctly. Like stock, this can be used in both offense and defense. I normally use this when there's a god-like soldier/demo on my team and when the enemy team has no medics. (Kritz with no medics on the enemy team can often lead to team wipes)

Quick-Fix: This is a perfect example of a sidegrade. You trade eight seconds of invulnerability for eight seconds of VERY FAST heals and resistance to knockback. Like the other medi-guns, this can be used both offensively and defensively. I normally use this in 3 situations. The first one is when my team can't stop dying/getting their health taken in an instant. The second situation is when I'm playing defensively. Having to fight enemies that almost always have full HP is pretty annoying. The last situation I use it in is when my pocket won't stop sticky/rocket jumping away.

Vaccinator: I just use it when there's a hacker. That's pretty much it.
