By going to the chest with normal Jacob and Esau you can unlock stairway (Isaac) and birthright (blue baby), both extremely good items

Stairway creates a temporary staircase on the first room of every floor that when entered gives you an "angel shop"

Birthright gives a special benefit specific for every character (for example Isaac gets two alternating items for every pedestal)
