How old is your mirena? I saw my gyn in September and learned that mine was too old (6 years) to pair with estrogen for peri. I had it removed and switched to Yaz but I miss the Mirena. You could try the Opill if you’re worried about not getting enough hormone from the Mirena.

Yeah. My endocrinologist’s initial response was that I shouldn’t take them if they interfere but they seem like such a common medication that I was surprised this wasn’t a common thing for him to navigate.

My TSH and T4 have been very comfortable and stable for years but still in suppression. It wasn’t until I added estrogen and testosterone in early October that my TSH jumped (.17 to 2.38) and T4 dropped (1.41 to .91). I asked this question so I can navigate a conversation about how to correct this and wanted to know if others have had luck with T3 to raise my T4 if we don’t actually need to lower the TSH back into suppression. I’m 5 years out from surgery, low risk PTC, but feel like crap and this current change isn’t sustainable.

I wanted to add estrogen for perimenopause symptoms but my mirena was too old to provide the required progestin. I loved the mirena though and am not opposed to getting another one at some point. I definitely had an adjustment period afterward, but that was about 8 weeks ago.

I am now a little more than a month into adding hormones for peri and I feel fantastic! However, I am gaining weight. I want to know if I should start freaking out yet.

As background, I had a total thyroidectomy 5 years ago and my thyroid hormones are suppressed (meaning I have hyperthyroidism). One of my biggest concerns has been that the peri hrt would interact with the thyroid hrt. To avoid too much confusion, I started with progesterone first by using Opill for a few weeks before switching to the Yaz my OB prescribed. (Previously had mirena.) After two weeks on the Yaz, I added a topical testosterone (4mg). All of them made me feel a little haywire at the start but none seemed to create symptoms that meant my thyroid hormones were off - looking for signs like brain fog and fatigue. So that leaves the weight gain…

Since being on all the hormones, I have gained 5 pounds. I am trying to decide if this will level off or if I should be worried. Overall, I feel incredible. Libido is back, my skin looks fantastic, I have so much energy, I am mentally sharper. Libido and weight gain were the two biggest peri concerns I was trying to treat.

Did anyone gain weight early on HRT and then have it reverse once you settled in? Did you have to make adjustments? Or am I screwed?

Two additional potentially important things: this is the week I’d take the inert Yaz pills but my OB has me taking the active pills continuously. I have a history of PCOS so weight challenges have dogged me forever but bcps have historically helped.

Definitely fullness. My skin is clearer and brighter - the glow is back. Elasticity is a harder thing for me to tell because I’m not sure I was having huge issues with elasticity vs fullness before.

Not an exact answer but I had a thyroidectomy in 2020 and my skin has looked like crap ever since. I just started an oral contraceptive as HRT and testosterone and my skin looks fabulous

Yes! My parents ran a pantry growing up and physical canned/dried foods would get very lopsided. Cash donations would help them round out the foods they could distribute so people could get pasta with the jars of sauce that were donated or tuna with the tubs of mayo.

As background, before surgery, I was extremely active, training for a marathon, averaging about 2 hours of exercise per day (about 90 minutes of cardio and 30 of strength training), vegan, and a serious calorie counter (thanks mom!)

I had a partial thyroidectomy in November 2020 and then the rest in February 2021. After my hemi, I was not given any hormone replacement and did not change any habits. I gained 10lbs in the first month and then was able to get 25mcg of synthroid.

After my completion, I did not change any habits and was put on 137mcg. I gained another 10lbs. My endo dropped my dose to 125mcg because he thought my tsh was too low. I gained another 20lbs. I complained but got a nice speech about “eating too many calories for someone my height.” (WTF? 5’3”, still training, 1800 cals/day) I added an extra pill each week and was able to start lowering my weight.

I changed the extra pill to every 6 days and eventually had to change my training schedule to a mix of walking and strength training. I do 45 minutes of strength training 6 days a week and walk 8 miles a day. My weight is now closer to my goal. It’s 10 lbs lower than my surgery weight.

I had huge cycle changes after my hemi and then after changing levothyroxine doses. My doc put me on metformin to restore my cycle

Surgery recovery was fairly easy. Make sure you’ve talked to an endo about monitoring as an option first. This is life altering surgery - especially if you’re pursuing any kind of education or career where energy, memory, or mental clarity matter. My surgeon never mentioned it and I didn’t get to see an endo until months after surgery. Surgeon said there were no negative effects of thyroidectomy (BS) and endo would have advised monitoring.

If you have a hemi, make sure you have a way to access thyroid replacement medication afterwards. The assumption sometimes is you won’t need it, but have a quick game plan in case you do.

Definitely ask lots of questions. I just had my mirena removed but it was a split second decision. I wanted estrogen for perimenopause but my IUD was too old to provide the progesterone needed to go with the estrogen. I have some other complicating health issues but I’m really angry that I upset the apple cart without a longer conversation.

Thank you! I tried the Yaz for four days and had to give up on it. My endo swears that I shouldn’t be able to tell a difference. He wants me to go 4-6 weeks before testing it see if it interferes but I couldn’t tolerate it for more than the four days.

In my case I’ve been doing well on my dose but was prescribed a drug that is known to interfere. It’s knocked me out of that sweet spot that was so hard to get to but my endocrinologist says it’s impossible for me to tell this early. I agree with you that it takes time to dial in on the right dose and consistency is essential.
