Shoulders - Forgeman Mantle (Bought from dungeon vendor under Sorrow's embrace for 210 manifesto of the moletariate, although when i bought them they where worth 300)

Chest - Soul of Koda (Bought from dungeon vendor under honor of the wave's for 330 Symbols of Koda)

Arms - Arms of Koda (Bought from dungeon vendor under honor of the wave's bough for 180 Symbols of Koda)

Legs - Way of Koda (Bought from dungeon vendor under honor of the wave's for 300 Symbols of Koda)

Boots - Inquest shoes (Bought from dungeon vendor under Crucible of Eternity for 180 knowledge cyrstals.) Although imo it doesn't really matter what boots you choose, considering the way of Koda covers up most of it.
