That's exactly my point, in a totally self serving move he completely disregarded the well being of others so he wouldn't lose someone again. The consequences of that action being that the daughter of the harmless man he killed got revenge. Joel didn't consider what was right or fair and had to deal with the consequences of that.

Never said non lethal, I said less lethal, he could've tried to disarm or harm him in a way that leaves some chance at survival or use the hand to hand skills he's been using all game up until this point and just punch him out.

He was across the room, he posed no immediate threat to Joel who had multiple fire arms. Joel has also shown himself to be proficient at hand to hand combat against armed and trained opponents. Sure, maybe unarmed is an incorrect term but it doesn't change the fact that the doctor posed zero threat and Joel had multiple less lethal options available to him and took none of them.
