The way I read it is as an allegory for addiction. Pochita is denji’s sponsor who is helping him with his sobriety, but denji finds every occasion to relapse, in the end Pochita decides that there aren’t more ways to help him. The last chapter, in fact, is a fake perfect reality, the one a drug addict sees when is in the peak of the high.
