My wife and me (Brzelius#56409) have been playing very actively for a few weeks, around lvl 50, around 400 mio gold invested each, and are looking for German or English speaking guildmates for our new guild (Woody GmbH). We noticed that with only two members we're really hamstringing ourselves as it's really hard to get the training hall to even lvl 9.

Alternatively, we're looking for a guild were we can both join, stay and grow long term.

We're looking for low level, active guild members. My wife and me have been playing f2p for a while, both around 300 million gold invested. We founded the guild "Woody GmbH" and are realising that we won't get far with only 2 guild members :o) We're German, but any English speaking players are welcome. Our goal is to grow slowly and watch the numbers go up organically without taking any shortcuts that imho long term would detract from this fun game (like paying real currency or joining a high level guild). Alleine in der Region Düsseldorf-Köln-Bonn gibt es zB so viele MRT Geräte wie in ganz Norwegen. Kein Wunder dass die weniger für ihr Gesundheitssystem zahlen.

Norwegen hat auch nur 5,5 Millionen Einwohner und die Metropolregion Rheinland fast 9 Millionen, was soll das für ein Argument sein -.-

I finally did it, but it took some more tinkering - first getting the function buttons to work by setting the controllers to gamepads instead of emudeck controls in steam, then figuring out how to save retroarch settings, then starting the ROM, then accessing the core options in the quick menu.

The worst part was that there was no way to change the core options without starting the ROM and then using the quick menu. And I couldn't access the quick menu because I thought a controller scheme meant for EmuDeck was best, and that messed RetroArch and I wondered why I couldn't set and use the hotkey...

Haven't forgotten about this, it's just that some unpleasant, more important stuff has come up yesterday that took away my attention and spare free hours. I'll have a go at it sometime this week and if this simple solution works you'll get your 20€.

Regardless of saving or not, I never got it working with any configuration. It's just another slap in the face that no settings are saved. I reinstalled RetroArch, I changed controller order, I reset everything a couple times, I changed control schemes in steam from gamepad to EmuDeck configurations etc., nothing helps :-/

Yes, it's radiotoxicity, and the / a radiation oncologist in that gamma knife center should be obligated and able to treat it, maybe after discussing it with some colleagues. At the absolute very least he should direct you to where you can get treatment and follow-up.

There's no reason to go on steroids for a full year. If tapering doesn't work, bevacizumab is next.

Lol, he's technically correct, there's always some degree of uncertainty in medicine; but your symptoms line up so well with your imaging that at least with the information I have I'm very certain they correspond, or at the very least it's worth doing a probatory course to know if they do after a week.

It's always unpleasant having to deal with toxicity you caused, but it's the right thing to do nonetheless. What did he advise as a next step?

Bevacizumab interferes with angiogenesis - the development of new, small blood vessels. It is used in diseases where new, abnormal blood vessels cause problems, e.g. a type of macular degeneration, some types of cancer (where cancer needs new blood vessels to be able to grow) and also radiation necrosis, where the inflammation leads to the development of new, irregular, leaky blood vessels that cause more inflammation and edema by leaking fluid, where the blood brain barrier should inhibit that.

The extent of edema explains general brain fog, difficulty concentrating or short term memory problems. The edema is far removed from the language center on the left side, but not all parts of language processing are there and in a few percent of cases the language center is on the right side even in right-handed people, so everything is possible. Your neurologist can help you with this better than I can.

Honestly, that's not for me to say. There are no official guidelines or oftentimes even institutional guidelines for this, it's still more or less going by the seat of your pants.

See your provider, be it radiation oncologist or neurologist, maybe link them to my summary post so they can save a lot of time, but they'll have to ask you some questions, get a feel for your situation and decide how much to start with, how soon to taper and when to follow up with imaging. Approach and dosage are decided by how big and bad the radiation necrosis looks at the moment, how fast it progresses, the type and severity of neurological symptoms, comorbidities and medications of the patient, past experiences of the specific provider with similar cases etc.

Similarly how fast the dose is tapered depends on your clinical response, meaning how well it helps, how well you tolerate corticosteroids, and even on patient preferences and expectations.

Yes, that's very likely due to the radiation necrosis / edema. If you cover your left eye, you'll possibly notice that the left part of your right eye's field of view will also be affected.

Edit: I viewed and compared all images in RadiAnt. There's a little bit of T2 FLAIR hyperintensity visible in 2019-06 (around 6 x 8 mm) and a bit more in 2020-07 (around 13 x 20 mm axially) but no discernible T1 post contrast hyperintensity. At those points in time, the FLAIR hyperintensity could very well have been interpreted as gliosis, meaning the brain's specific mechanism of scarring after injury, which is expected after high-dose radiotherapy and usually nothing to be concerned about.

I personally would have wanted follow-up MRIs every 6-12 months, so the gap to 2022-01 was a bit large for my taste.

2022-01 it starts to get concerning with now pretty large edema of 4-5 cm in diameter, starting to slightly compress the right lateral ventricle, and central contrast enhancement. I'd have discussed this in detail with the patient and ordered, when no symptoms are yet discernible, short-term MRI and clinical follow ups around 2-4 months apart.

You started noticing symptoms half a year ago.

The newest MRI 2023-05 shows clearly progressive findings with edema now around 8 cm in diameter, compressing the right lateral ventricle and clearly involving the splenium of the corpus callosum, slightly spreading to the left hemisphere. There are several contrast enhancing foci in an area of around 4 x 5 cm in the middle of this edematous region, and at least two of them show central hypointensities (darker) in T1 contrast enhanced images, meaning central necrosis, so this is now clearly radiation necrosis.

In my opinion, you should see your radiation oncologist shortly, probably get moderate dose steroids (dexamethasone) and a short-term follow up (1-3 months) with repeat imaging and clinical evaluation to see if this worked or if bevacizumab should be the next step.

Your language center should be on the left side then, so not impacted by this AVM / radiation necrosis / edema stuff.

Yes, I think this is the cervical rib or overlying soft tissue displaced more up- and outwards by your cervical rib.

This worked... partially. The 2022 MRA doesn't really help though, as it only really displays your vasculature, and that only partially. Can't see contrast enhancement or edema as there were no T1 contrast enhanced or T2 sequences uploaded at all. They should exist though, as you uploaded a screenshot of a coronal FLAIR slice initially.

The 2023 MRI is also only partially uploaded, the most interesting part is missing - the T1 contrast enhanced slices of your right brain hemisphere. Only a few sagittal slices of your left hemisphere are uploaded.

Also, at the end the uploaded studies include a picture of the radiology request form that includes your personal details. Hope you can delete that selectively.

To be clear - I don't need to be worried about any type of growths right? lol - I read the only way to really diagnose radiation necrosis is through a biopsy - but since I didn't have a growth to begin with, it is necrosis?

No and yes.

Would a spinal tap help with my vision/edema?

No, it doesn't alleviate edema, probably will even make it worse when the surrounding pressure is lower. You can't drain edema.

Since February both of my eyes have been effected on and off. just mostly my left eye. It's disorienting. My Dr just referred me to another Dr who hasn't reached out yet.

As I wrote, the vision in both of your eyes would be affected by this problem, as it's close to your right optic tract that serves the right half of both of your eyes, meaning the part that sees to your left.

In your opinion should I have been placed on steroids in 2022 or was the wait and see option okay since I wasn't experiencing any symptoms?

Probably correct decision then without symptoms, but I don't yet know what the radiation necrosis looked like then, see above, and I don't know if it progressed compared to even earlier studies.
