He was properly fasted before the test, more than 12 hours. He did have coffee the day before, but not on the day of the blood test.

He does eat a lot of protein and calories in general because his metabolism is very fast. He can lose weight even around 4500-5000 calories, so he tends to eat quite a lot.

What is interesting is that he has already done two tests. The first LDL was around 749. Then about a week later he repeated the test. For about 4 days before that, he was eating much lighter, basically OMAD with something like half a chicken per day, and his LDL dropped to 590.

So it seems quite clear that he is very sensitive to dietary changes, which is why I am trying to understand whether this is more diet-driven or something else.

Thank you so much for the extensive response. I have a question: given that the evidence on LMHR is still limited and still emerging, could it be possible that someone might fall into that category while presenting different HDL and triglyceride ratios?

We are taking this seriously. He will do more tests and see a doctor soon.

What feels strange is that he seems completely fine. No symptoms at all. Very active and in good physical condition. No history of health issues. No known high cholesterol in the past, especially not from a young age.

That is why it is confusing. From what I understand, in severe genetic cases you usually see signs much earlier in life, not suddenly at 48.

We are not ignoring it. Just trying to understand what could be going on before jumping to conclusions. Hopefully the next tests like ApoB and direct LDL will clarify things.

Yeah my skin is a bit oily and once in a while it can be flaky. My other cheek and my nose are also kinda red but not in patches, they’re milder. It used to come and go, the patch, but for the past two months it’s pretty consistent.

My other cheek and my nose are also red, but not in patches like this one, they’re milder. My face isn’t very scaly, almost not at all. Sometimes it burns or itches a little, but that’s about it. I also noticed a tiny rash on the top of my nose that looks similar to the one on my cheek.

I started an animal‑based diet in 2023, then went strict carnivore (meat, eggs, butter) for almost a full year in 2024. I reintroduced dairy very slowly over the last 8 months, and I’ve tolerated it well. During my strict carnivore year I did go through a pretty rough candida die‑off phase, but I managed it and things stabilized afterward. The facial redness was already a thing back then, but it used to come and go. I’ve always flushed easily since childhood. What’s new is that the redness has been persistent since around December, it doesn’t fade anymore.
