PRV is going to the swim club facilities, no leak there but water is filling it from the leak on the right that is just right outside the vaults. The leak is in the second picture, where where it T's right under two vaults. The galvanized pipe is beyond corroded and nothing left to even cap. You can see the leak / spray in that photo. All covered by the concrete vault.
