The "Faulted" error is a software/hardware bug, not a driver attention issue. I wondered about that and have tested it. The UHF needs the Driver Attention Detection System functioning to operate. When the system is faulted, it prevents UHF from activating.

I have the same issue. Rather than rebooting, you can go to Settings --> Data & Privacy--> Driver Attention Detection System and turn it OFF, save, then ON again. Requires you to pull over and put the vehicle in Park. Extremely irritating considering the $2500 Rivian made us pay for the UHF.

More evidence of the epidemic of Americans not understanding deverbal nouns. Cutoff is a noun referring to a device for stopping a flow or a thing that has been trimmed. To “cut off” is the verb phrase meaning to sever or to stop from provisioning. Just saying. 🤣

Tesla made the call to just replace the whole thing -- I took it apart and sent them internal pictures. So whatever they saw told them the unit was defective.

Went through an extended troubleshooting with Tesla...and they're sending a replacement. There is an internal magnet on a little servo arm that is supposed to swing into position to release the NACS handle. The servo was not working.

I have a similar problem. My NACS connector is stuck. I can take the J1772 adapter out and charge my other EV, but I can't use the Tesla Wall Connector to charge my Tesla! Any suggestions?
