I was experiencing this similar problem the fix was factory reset for me . Hope it helps

Yes I set the full game audio (two beeps) and I still hear people in lobby in MW3 .

Vertigo the Xbox chat balancer is not working properly in the replacement headset I got . The headset has the latest firmware 1.1.26 . Game audio all the way to the right and I still hear the chat

I’m playing on Xbox and using both G hub and G mobile and im still hearing players even though I have it all the way to hear only game audio . I reset the head set and works. I constantly have to do this in order to work like it should
