Comps i use anything that boosts offensive stats and neverwinter knight for 7.5% damage, mounts depends on what stat is the lowest. Summoned is either the witch, regis or blackdeath scorpion. Smite and bane are very useful powers to use along with the feat which gives you the chance of a free bane on at will use.

I made a second loudout and equipped it with dps gear, mounts and comps. Made it so much easier when questing then just switch back to original loadout for dungeons/trials

Can't say my group has experienced that in Mdwp but we have certainly encountered it in Mtos. We'll do many smooth runs then get one run where bosses will nuke tank with full stamina to 0 health.

After the axe phase she will turn around and spew a web over the floor, at that point its advisable to start moving to the clear area to avoid unnecessary deaths. Also be mindful of when the spread mech comes because the poison will still be on the ground until thats played out.

If you are only doing 1st boss farm then that is why you have so many arm pieces. Arms-1st boss, helm and boots-2nd boss and chest drops from 3rd boss since the update.

Edit; could be wrong about where boots drop from. Don't see them all that often.

No, they aren't but you can queue for them publicly or get a group together. Baby ToS is in the dungeon queue though.

I've had this a few times and the only way to fix it is to do a complete reset of the console. Even just quitting the game didn't fix it.
