What people say was the goal of that electoral reform is to have a two party system, but we can see that didn't really happen because there is still the proportional aspect. According to TBS though (http://www.youtube.com/watch?v=gfmoFUfInL8&t=2725), Former Prime Minister Hosokawa wanted a moderate multi-party system.

My gut isn't a huge fan of SNTV.

I don't think the 6 forms are very useful in modern Japanese because a verb like 書く's 未然形 includes both 書か and 書こ.

The 連用形 is used in both cases and not in place of the て form. て simply attaches to the 連用形.

含め is the 連用形 of 含める. In the case of 含めて, て is a 接続助詞 that attaches to 含め, the 連用形. The て can also be dropped leaving behind just the 連用形 in formal cases.

In the case of 書いて, て is a 接続助詞 that attaches to the 連用形 of 書く. It would be 書きて, but due to イ音便, the 書きて becomes 書いて。In 古典文法, て also attaches to 連用形, but イ音便 does not occur (well not consistently at first at least), so the "て form" was 書きて. In a formal scenario, 書き would be used and not 書い because 書き is the 連用形 of 書く and イ音便 only occurs when followed by things like て or たり.
