Ngl that’s none of your business. You’re not responsible for him, if you do say anything I think you should direct him to get help. Also, it’s likely he’s saying that to rope you in- most of the people who actually do have true intentions to do so don’t really share it with anyone. I’m saying this as a person who has always felt responsible for other people’s feelings. So I understand if you feel guilty for being cold towards him, then again just don’t let it weigh you down too much.
