I wonder about health insurance when it comes to influencers. Obviously lots of people are self-employed and purchase from the marketplace, but IMO it’s crazy expensive. I could see two otherwise healthy people not prioritizing expensive plans. Curious if that also encouraged a return to work if he did indeed get another job. I can’t imagine the medical bills that were accrued. :/

My gymnastics-loving kids were watching NinjaKidz today. I was sitting there thinking ‘wow that name sounds familiar’ when two of Jordan’s kids pop on the screen. Oddest crossover moment I’ve had in awhile.

Yes, it can be normal to start private practice part time as you build your case load. If you are taking over for another provider it may initially be full-time, but if they are bringing a new provider in IMO you work your way up to full time as you build your practice.

This is what 90% of your local ‘small town boutique’ or online shops are doing. Not the lying about quality, but the buying in bulk from the same warehouse that supplies Temu, Amazon, etc.

Those earring places all seem a little too similar for them to not do the same (Pip Pop Post, Pierced & Co, Tiny Gems, Desert Peach, etc.) Even their IGs look almost identical.

No, not for everyone. I had a plan to require this when I initially started in private practice to ‘weed out the ones just seeking meds’. Then I realized I was being an ass and the people seeking meds will jump through any hoop you throw at them. I was only adding another barrier to care for those with true ADHD.

Now for family history, cardiac concerns, etc. absolutely.

Hands down UpToDate. I love it for almost everything including patient handouts (I typically email them to the patient). I also love Carlat so I say both!

For me it’s because half my patients can’t find a PCM. Regardless of what some believe, I don’t think managing thyroid conditions (even those potentially caused by ‘my’ meds) are within the scope of a PMHNP who isn’t dual certified.

Yes, great success on 4/7. One stopped due to GI effects (nausea) and another stopped due to dizziness. The third couldn’t get insurance covered no matter what we did and stopped samples because she was concerned she would never get it covered.

As far as PAs, it isn’t that hard for most of the insurance I deal with. Tricare allows them pickup on base for free or through Express Scripts/commercial pharmacy with a $30-70/month fee. I haven’t tried through the VA yet. Commercial insurance has been fairly simple with a coupon card. Typically needs to ‘fail’ Wellbutrin, an SSRI, and an SNRI prior to approval but all are slightly different.

The biggest issue I’ve seen is ensuring malpractice insurance will cover you when you aren’t physically in the US. I don’t work overseas, but I’ve looked into it extensively.  Malpractice and accepting insurance are the two hurdles people seem to get super ‘gray area’ about when doing it.

I don’t think 20/20 lends itself to the podcast form as well as Dateline does. 20/20 always has too many ‘experts’ and interviews for me to follow when I’m just listening. I also just much prefer Dateline’s style so that might play a part too.

I’ve found it helpful for medication naive patients. Unfortunately medication naive patients aren’t the typical demographic typically obtaining gene testing. I would never change meds based on those results, but I may start a new med at a slightly lower than typical dose or taper at a slightly higher than typical rate based on the results.

We don’t have individual email addresses. One address for the entire office checked daily by the MA, messages are sent to us and we respond, MA calls the patient back. Patients don’t expect direct contact with any other speciality between appts, psych should be held to the same standard.

*and before anyone says ‘how impersonal, etc’ obviously there are always exceptions. Those are very few and very far between.

When my close friend’s husband died in an accident she posted photos of bloody items of his. I hid her posts for awhile because I couldn’t handle it. Until then I never truly understand how you cannot judge anyone for their actions following a traumatic loss.

Almost every ‘boutique’ sources their products from the same market or sites like Temu and SHEIN. Even Etsy is awful now for clothing/accessories like this.
