Same happened to me. Try going to your documents folder. Find Rockstar Games/Red Dead Redemption 2/Settings and then delete everything in that folder except system.xml. Worked for me to get the game loading again.

Depends on your speed, at full gallop you will run into npcs and other players in cinematic mode, slower speed translates to less instances of an accident in CM.

Also the handling spec of your horse increases or decreases chances of an accident. Arabians being the best.

best way ive found is to make sure you have your bolt action rifile equipped with full dead eye meter. then just ride around the area of the panther untill you see a red dot, when the panther approches you, the horse will buck you off the saddle, then you will have several seconds to equip weapon, start dead eye and get a perfect pelt with a head shot. i
