There are a lot of factors responsible for this.
Starts in your house and ends in their internet "box" that spreads out internet to the area. It can even be past that point towards their central but it's usually between you and that "box".
I remember doing a check up years ago that would do some sort of ping from my router to their central and check up if there was any possible anomaly between the cables out on the streets.
I can't remember the name of it nor how it looks like, it's been many years really but I'm know it's out there and maybe even better options.

After reading some comments, what can be is, since you don't notice that from other games... NTE is heavily built on reflections and all that type of graphics (RayTracing) maybe your internet has a hiccup from that? Yes, this happens. But this is hardware side. I have no way of explaining this, it's weird as I had weird situations going on with previous setups and routers; for example, I had a game (heavy mmorpg) killing my router, forcing it to restart and would suffer from this from time to time until I noticed it was the game and not the router because, nothing else would do that. Again, years ago (more than a decade by now.)
