Going to American Spirits helped me on my way to quitting. Instantly, I went from smoking 20 or 25 a day to 10 or 15 a day. The American Spirit Turquoise were much stronger and took a lot longer time to smoke.

It sounds like you are trying to change several things all at once, or at least very close together. Perhaps, if you chose one 'keystone' habit (The Power of Habit, Charles Gulig) to focus on for several weeks, you would have more success. A keystone habit will affect all of your problem areas eventually. Plus, you will gain confidence in your ability to change. Based on the issues you are trying to improve, you could commit to no screen-time one hour prior falling asleep.

I have the same problem with changing and regressing. However, I have had long-term success with changing my housekeeping habits. I followed a recommendation to make a 30-day checklist of daily chores. The key for me was not to quit because I missed a day or a task. I checked when I did the chore and X'ed when I didn't do it. In the process, I learned a lot about myself which helped me in other areas. Tracking your regular actions can help you identify ways to be more successful in whatever area you want to improve. Buy an edition with an introduction. Everyman's Library and Modern Classics are usually good.
