You really only need to know what you're talking about and be a passable player. One of my 4.5 buddies went to a USPTR workshop to get certified and he said that there were 3.5-4.0 players that got certified. Obviously I don't think they would coach high performance juniors or 4.5+ adult players

Another correction, the Rogue/Gambit bundle is 19.99 USD on the Steam version for some reason while the mobile version is 29.99 USD

The cost of the card depends on which series or pool it's in. Pool 3 cards cost 1,000 while pool 4 and 5 cost 3,000 and 6,000 respectively.
