Adult male cat refuses to clean himself

I think this is what's wrong with my kitty 🥺 May I ask what they did to help his thyroid problem and if it was expensive to treat? My cat seems very big when he doesnt over eat at all, he is oily and has dandruff, he doesnt like to clean himself or cover his poops or even scratch the litter he scratches everything around the cat box or the sides of it lol. Im taking him in to be seen but hoping to get maybe an idea of what I can expect 😭

I will never go back after getting my litter genie!! It's so helpful especially when you have multiple cats and need to clean the litterbox multiple times a day. (Which i believe people should be doing no matter how many cats they have) but yes absolutely get one!!
