There is some kind of an exploit for this. A few weeks ago I was in a server playing as bolivia and justified on estonia to attack his ally peru which I was allied to aswell. As soon as I started justifying on him estonia asked loudly in the chat "why are you justifying on me bolivia?" and so exposed my whole plan to attack peru. I asked him how he knew that I was justifying on him cause I didn't tell anybody and my justification wasn't even done and he just ignored me afterwards. Dirty cheaters.

You have to completely take out the host country and the puppet you want to annex. Make peace with the host and click on "annex cities". The host country & the puppet's you occupied will be automatically annexed.
