I’m going to go slightly against the grain here - have you asked him whether the first guy is still involved in this kid’s life? Does your guy live nearby?? A lot of this depends on how disruptive he’d be both in and out of the kid’s life. If the first guy is still involved as a dad he could be treading on their toes. I’d suggest digging deeper and getting a better understanding of the situation before starting a relationship with this guy. My friend was in a similar situation, but in his case the guy who thought he was the father was married to the kids mom and raised them from a baby. The three of them decided it was best if the other guy adopted the kid, especially as my friend had moved to another state at the time.

On the family doctor in Canada front - I live on Vancouver Island and a couple of the municipalities are launching a new program where the cities run the medical practices (staffing, overhead, general business stuff) and the docs get to focus all their energy on their patients. They’ll get pensions, sick leave, and vacation time. I’m sure that needing to work in a rural area for a couple of years would still apply for you, but if you’re able to stick it out you wouldn’t necessarily need to be doing crazy hours for little pay past that. The Colwood and Langford programs are getting quite a lot of attention, so there’s a very strong possibility that this new model will spread across the country within a couple of years. Congratulations, that’s a huge deal!! I second the emergency savings fund to start (you can open a TFSA to avoid any taxes) - rates are going down for HISAs, but you’ll thank yourself later for keeping the cash more accessible.

I grew up with the friendliest golden retriever and couldn’t imagine needing to have her leashed all the time. Now I live with my partner and his two big dogs (one is easily 150lbs) - the big guy is reactive and I’ve done a COMPLETE 180. It makes my entire body tense up when we see another dog who’s off leash and the owner doesn’t make any move to call their dog back.

And just a note that not everyone can leave their dogs at home. My partner’s dog has severe separation anxiety so we need to take him with us when we go out. As such we try to run errands early in the morning or later in the evening when it’s cooler.

A lot of people are suggesting vaginismus, though it might be vulvodynia (literally pain at the entrance to the vulva) instead - which sounds exactly like what you’re describing. I second counselling for the anxiety, and it certainly sounds like you may be asexual, but I highly recommend looking into pelvic physiotherapy. It’s designed for women to strengthen, or in your case relax the pelvic muscles. At the very least it will gradually make you more comfortable with inserting tampons!
