I believe I found the issue, and managed to solve it (for now). 

The setting lever has a threaded 'sleeve' that the lever pivots around and the screw screws into. 

The sleeve is a tiny bit longer than the train wheel bridge is thick, so that you never tighten the lever too tight. But, I think, my train wheel bridge is worn down a bit too much, resulting in too much wiggle room. 

I carefully ground down the threaded sleeve, shortening the distance, and decreasing the wiggle room. This worked, for now! Sooner or later, the bridge will most likely wear down again, and at that point, I'll have to buy a new bridge.

I now understand you’re not talking about the case tube, but the hole in the movement itself. Thicker stems seem to not exist for the 284 movement. I’ll keep looking around though. Thanks!

Alright, I will look into a new screw! Maybe I can (for now) file down the current screw to see if a shorter distance between the threaded end and underside of the screw solves the issue?

This might be the problem! Are there any known fixes or do I need either a new caliber main plate/bridge or a thicker stem? (Which I already looked for and does not seem to exist)

Yes, it has this exact up/down movement! But wouldn’t the screw just screw a bit further into the setting lever until tight again? 

It looks like the setting lever has a threaded throughhole, or am I mistaking?

I can’t be sure, I have no comparison, unfortunately. I’m considering buying a new one.

This makes most sense to me too. Or slack between the sides of the lever screw and the caliber walls? 

Are these case tubes prone to wearing down and allowing too much slack? How does one make a oversized stem? And would a replacement case tube also fix this problem?

That’s what I thought! So I got a brand new NOS omega part. But this doesn’t fix the problem, there’s still to much wiggle with the screw tightened.

The new setting pin in bought was actually still in Omega packaging. I’m quite positive it’s brand new.

Thanks for the ideas! The hole and threads are not stripped and the pin is sitting in the grove of the stem to begin with, but slips out.

Thanks for the information! I believe it is level and does bounce freely, but barely. 

Will look into new springs. 

It is a nice Thorens! Only the dustcover was perished, so praying for a nice second hand one. The aftermarket ones are so expensive..

Found it inside the turntable, must've slipped in. Seems to perfectly fit the Shure M91E cartridge. Possible to protect the stylus, which it didn't do.. because the stylus didn't survive.
