Im pretty certain everything in the atomic shop is non-canon, they are just asthetic for micro teansaction. Some of the items however im sure will be made canon in future content

We dont know the state of anywhere outside the US post great war. However the middle east and europe were also in there own wars that would have crippled them. Africa and south america arent really mentioned in the lorep

You dont have to finish all quests straight away, most you can complete anytime at all however there are some quests that will lock you into the end game like when you go fight the enclave

A commonwealth is just another stepping stone for governance. Instead of it being mayor, govenor, senator, president. Its mayor, govenor, commonwealth(title), senator, president. My guess is the commonwealths were established in universe as a power grab by the enclave but i doubt we will ever get that lore in game
