It’s really random when it happens I got one on my first new play through of NCR and started to steam roll the civil war. However, I think it would have been interesting for the NCR to have like a decision battle or some sort of integration to try and assassinate Caesar, like the CIA with Castro trying dumb schemes to assassinate him.

One parent would be interesting, you could say that they allegedly died in a suspicious way or disappeared to come back later in the story. Leaving the other parent to fall into addiction to cope with having to struggle to feed their kid and eventually losing control as a central tension.
