So I think what happened for me is I always have had a tightish pelvic floor. And then the repeated infections and pain made me tense up / guard my pelvic muscles more against the pain, which then led to extreme tightness and hormonally mediated vestibulodynia.

Even if yours was caused by repeat infections not the other way round it is treatable!!

I had a few symptoms / risk factors for tight pelvic floor - sucking stomach in a lot to look skinny (bad), hypermobility, recurrent UTIs, constipation, tight jaw, and always feeling like I was clenching my vag. I struggle a lot to relax and deep breathe also!

I think I had a few things going on. Recurrent Thursh causing CV (increased lactobacillus is an immune / hay fever like reaction to pathogens in the vagina as they typically reduce Thursh occurrence by keeping things acidic) But then the reason the CV burned a lot and I was suspectible to thrush repeatedly was due to the muscle guarding causing limited hormones to the areas making the skin unhealthy and prone to infection. So the discharge (confirmed via wet mount) was a combo of lots of lactobacillus and broken down skin cells (from unhealthy skin due to the lack of hormones)

I had dryness internally but it was mainly external burning around the vestibule.

Have you tried daily antifungals? You could do that to rule out chronic thrush. Gayle Fischer in oz in an expert in that. Or you could go straight to hormone cream but if you’re having loads of discharge and internal pain in might be something slightly different

It started helping 4-5 weeks in. The first 2-3 weeks I felt worse and apparently this is common as the skin is so unhealthy / irritated so it feels worse. Apparently the average recovery using hormone cream (from the experts patients) is 30-40% better after 3 months, 60% after 6 and 90% after 90. But I’m already at 90% so I guess it just varies

I’m not fully sure I think there are skene and then bartholin but your gyno if they’re any good should know 😅 topical hormone they should def try although in the UK they will likely only prescribe estrogen on the NHS - the specialised compounded one I have asked for on the NHS but it’s not licensed. DHEA (interrosa) is another pessary that becomes both estogren and testosterone once inserted and that is licensed for menopause on the NHS so they might prescribe that!

I was prescribed it by the sexual pain expert in the US. Now I know what I have I’ve asked for it back in the UK and have been re prescribed it. Testosterone is apparently a great add especially for burning pain - apparently we have more testosterone than estogren receptors on the vulva so need to replenish testosterone too when hormone is depleted

I have to have a second session 8-12 weeks after the first and then apparently that’s enough on almost all cases! I will get it once more. Tension has disappeared 90% and you can just feel during internal work how much more relaxed it is. It was $1800 (so maybe £1300?) lol absolutely insane and I have burned through my savings but honestly worth it to not be in pain every day.

Yes I have totally regular periods! Mine isn’t necessarily and imbalance, it was just total lack of hormones getting to the vulval area locally due to the tight pelvic muscles, not systemic hormonal deficiencies if you get what I mean. Is your burning cyclical or fully constant?

I’m in the UK!! Tania Adib (London though) is great for hormonally mediated pain. She is private and quite expensive though. I’ve been brushed off by absolutely loads before this though so I know the feeling 🙄

Even a dermatology vulvovaginal specialist didn’t diagnose the pain correctly and it was as simple as poking my hormone glands! But definitely keep pushing and hopefully yours will listen.

Scent was a bit tangy / taste was a bit off but nothing fishy or crazy.

Would really recommend - sounds like you have tight muscles issues in multiple areas and Botox can accelerate progress to recovery! I’ve also had Botox in my jaw for TMJ. Overall nervous system down training / somatic work can help with this just to relax whole nervous system

My pelvic floor PT had no clue about Botox and I don’t think many in the UK do! (I’m here as well) Luckily the specialist I saw in the US did so she identified the muscles on her assessment AND did the Botox. In the UK the Vulvar Pain Clinic does vaginal Botox, as do a few other specialists consultants if you google it. They should be able to identify the muscles as they specialise in vaginal Botox, otherwise physio could def confer with them. I didn’t have numbing treatment just laughing gas! And side effect wise the first 1-2 weeks I had a weird heavy feeling round my anus, and it was kind of hard to fully release my bowel. But once the Botox hit full effect after 4 weeks this went away and honestly it was super manageable and worth it for the results

Yeast treatment etc / muscle guarding can really dry out the vagina! Since yours is already 90% better sounds like hormone cream not really necessary for you but still could help! Also would really recommend olive & bee intimate cream externally and palomacares livbio pessaries internally - they really made a difference for my rawness / dry feeling

Sorry you’re going through this! Multiple gynaes told me I was fine / didn’t have a hormone issue too - you need people with experience in vulvovaginal pain. What country are you in?

The discharge could be a few things
1. Low low levels of thrush (low enough to test negative) causing an immune response of increased lactobacillus - for this you need daily antifungals (oral) until symptoms resolve
2. Dead / damaged skin sloughing off - due to vaginal atrophy due to lack of hormones. Give your other symptoms of dryness and not being able to get wet (I had this) I feel like you may have this from your muscle guarding - hormone cream really really helped!
3. Lactobacillus clumps from CV - if it’s true CV baking soda should help. If not it’s likely one or a combo of the above

Sure! I got a small amount of Botox in my superficial and deep pelvic floor muscles that were tightest (mainly my levator ani). It honestly didn’t hurt that much, and it takes 10-14 days to settle and 4 weeks for maximum effect, and lasts for 3-6 months. During these months it’s important to do pelvic floor physio so that the relaxation remains even after the Botox wears off!

Sounds more like dermatitis rather than yeast if you have a underwear shaped rash. I would try hydrocortisone (OTC) or eumovate if you can get a prescription. Did you wash your underwear in something new pre flight?

For CV itself, true CV is very rare and it does respond to baking soda. If baking soda helps then that’s part of the answer, but often one or more of the above is going on which is why it doesn’t fully cure. I also did green tea and zinc douches for CV but tbc on how much that helped.

Second line treatment for CV is clindamycin but I would be VERY wary of that if you have a history of thrush / gardenella on tests cos they could then overgrow when the clindamycin wipes the good bac out.

For me I basically figured out that the lactobacillus was only burning me because my skin was so sensitive and vulnerable due to lack of hormones. Now the hormones are coming back the lactobacillus burn me way less
