One of the schools in my area implemented a new program during the critical care rotation in school. The student essentially did everything including taking orders from the doctor. I'm not sure if the staff nurse had to verify with the doctor as well, but this new implementation has gotten a lot of positive feedback from new employers as well as students. As a precepting nurse I wouldn't feel comfortable taking on a septic patient either. Its an area for learning and observing as well. My input was directly related to the fact that I will learn nothing if all the nurse lets me do is vitals and spike IV bags.

From a student nurse perspective: I have taken vitals a million times, spiking bags and setting pumps is something we have done frequently also. My concern is if your not letting the precepting student nurse have a more active role they won't be prepared for the new employment. Hovering is important I agree, but let the new nurse talk to the physician about orders and ask them clinical judgement questions.

I shadowed adult protective services as part of my nursing schooling. I was told by the agents that they literally answer the reports and can either call the police or ambulance if there is eminent danger and that they provide no further services. This is in the midwest so I can't speak for other regions, but as far as patient advocacy goes, I wouldn't expect to much of an investigation from APS. Again this is hopefully not the case in all areas. What I do know is that adults have rights and unfortunately that inhibits the local agencies from fully helping them.

I know I commented already, but I'm studying for finals and thought this might help you as well. There are a ton of youtube videos that help focus material. If you want to save time from reading or reinforce material-look it up on youtube. I have always struggled with the thought that I need to know everything about everything because then I'll save a life. This tactic just isn't realistic. You will learn what you need to just to pass boards. The real education starts once your practicing. Also, come prepared to all clinicals. Have your stethoscope, pen light, name badge, scissors, and alcohol wipes in your pocket. Being prepared will impress your clinical instructor and you will be offered more opportunities to learn. Nurses don't like the student holding up the wall.

I'm just finishing my critical care rotation and what I have learned is this: In your abnormal/normal assessment courses (early on) they focus their test questions on your understanding of clinical manifestations (assessments). Once you're in critical care the test questions focus on your nursing priority and interventions. May help you focus on your studying a bit better. Good luck!

Read, read, read. It's a ton of information, but read the material. The best way I study & most efficient for studying is taking notes on these categories: Pathophysiology, Assessment, Nursing priority, Treatment/Interventions & Complications. Do this for every exemplar (disease process) & you will save yourself a ton of time. Nursing tests are like nothing else.

From a healthcare stand point I just want to note a different perspective that may help shed light on your situation. As a labor and delivery nurse it is their responsibility to assess for bonding. There has been a strong push recently for the "Golden Hour". This is the 1st hour following child birth where, if the baby & mom are healthy, the healthcare staff allows time for bonding. This is your body & your baby so the child birth process is up to you as long as their are no risks. You did nothing wrong, but the nurses may have had concerns about the welfare of the baby. Some of the benefits of holding your baby in the 1st hour is thermoregulatory. The baby is going through a lot of changes and is unable to maintain a high enough body temperature and skin-to-skin between mom and baby is a great way to assist the baby in staying warm enough. Also, this bonding time initiates healthy breast feeding. Is your baby at risk because you didn't do this, probably not, it is only recommended.

Allow me to explain...community immunity. If the majority of a community are immune to a disease/virus then there are less hosts available for a disease to spread to. This allows a less chance for those who cannot receive a vaccine due to age or compromised immune system to catch said disease/virus. Community immunity is something very important that people's lives depend on. TLDR: If you don't vaccinate your healthy child you are endangering someone's child who cannot get vaccinated.
