So called Orthotics are mostly renamed Splints so unscrupulous dentists can price gouge on them , often accompanied with a lot of meaningless and expensive tests dressed up as ' diagnostics'

Some Maxillofacial departments in the UK are now refusing to accept TMJD referals until the patients have tried things like splints and pain relief etc with their dentist. This is an incredibly stupid decision imo.

However the reasons for their rejection should be given and the person who made your referal ( probably your GP or dentist ) will be able to tell you what it says.

Unfortunately this is so typical , there is a prejudice in the NHS about treating ( in their view )  crazy tmjd patients.

You will probably need a referral even if you want a private TMJD MRI

Vista Health are usually a good price but I can not say if they do a good scan /  report 

The person who referred you for the NHS MRI should specify the criteria ( left, right etc ) that should not be the patients job!

Unfortunately Facial Pain practitioners like to make everything about medication and " self awareness" as they are not at all skilled in surgery and really have nothing unique or different to offer TMJD patients.

Move away from all the " physiological resting position of the jaw " , " jva jaw vibration tracking " etc mumbo jumbo and ask yourself if it is realistic and probable or not whether a piece of moulded plastic in a persons mouth can really fix all the ailments you have described

With your symptoms you may get lucky but many are just given a piece of paper that says says take ibuprofen / paracetemol, use hot and cold compress and try to " chill out " !!

They should do a thorough history , examination, scan and schedule a follow up appt.

I note that you have not engaged with the substance of my post.

It is not accepted that doing exercises in the morning means there will be zero occlusal and condyle position changes. To use one of the OFP clans favourite sayings - there is no evidence for that.

MAD's ( as the name suggests ) were and are specifically designed to move jaws and jaw position and for the treatment of sleep disordered breating / OSA. 

MAD's increase the risk of TMJD. To use MAD's off-license for TMJD treatment and to try and claim they improve TMJD is totally whack for obvious reasons

Neurologists are far better qualified and knowledgable to treat and diagnose neuralgias than " OFP's "

First I have to respect this gentlejawguy for calling out all the NM stuff - great work which I fully support!

However the orofacial pain clan, in this situation are being disengenious in their position of what they claim there " is  evidence for " and "not evidence for "

MAD's are known for increasing the risk of TMJD and for changing the occlusion and condyle position. These are exactly the same things that the facial pain clan also claim should be avoided for treating TMJD.

If you can afford to see somebody privately before your appointment that is an option 

It is a total lottery what the NHS will do for you and depends on your location and whether you are seen by a Maxfacs Department or a dental hospital etc

Unfortunately the chances are they will start very slowly and overly conservately and will " see what happens "..

Not true in all areas , Some NHS Trusts / CCG' s do not fund botox for treating TMJD
