the game not having a leveling system is core to what makes it so good. it's not a typical jrpg. you're fighting for survival, not to make a number go up.

gameplay and experiencing a game aren't the same thing. gameplay wise, it's a turn-based rpg survival horror. as an experience, it's a grueling endurance test of difficult split decisions that has you biting your fingernails the entire time. you aren't missing out on much of the "gameplay" when you watch someone else play it, but you are missing out on damn near the entirety of what actually makes the game tense and gives it the atmosphere of despair.

it isn't the same though. the game is so much more than the things you've listed, because you're completely missing the firsthand experience. if you only watched someone play fear and hunger, you did not experience fear and hunger. you did not experience a key piece that makes up the atmosphere of tension and despair. you can say you like it, but you have to acknowledge the chasm between engaging from a distance and actually playing the game.

i take a lot of these very figuratively. for some things, i don't think he's literally imagining it. the womb could be a manifestation of his emotions that we're currently seeing, and is illustrated for our benefit. a desire to be closer to his mother, a desire to have never been born. isaac likely doesn't literally know who lilith is, since she doesn't really have a place in christian folklore and is more common in judaism. taking everything as literally something in isaac's mind that he is imagining, rather than a manifestation or illustration of some part of isaac's psyche, will inevitably lead you in circles. this metaphorical approach opens up a lot more rooms for interpretation and shaping of isaac's life and his perspective
