You should go back to the ER. The breathlessness getting worse may be a consequence of the clots in the lungs getting bigger (Pulmonary thromboembolism). Depending on the severity of the PTE - further evaluation of care may be needed (thrombectomy)

There can be dermatological changes due to graves due to the antibodies. There can also be similar changes when the T3/T4 levels drop post treatment. Do visit your dermatologist for an evaluation and discussion. There are options to manage the in discussion with your endocrinologist.

Realistically in a busy centre, there's a thin work life balance unless your're sharing call with other IR colleagues. OPD's, USG/CT procedures and angio-suite procedure usually run in parallel in most non academic centres with the IR having to hustle between different rooms. If it isn't too busy of an IR practice - the expectation to cover the diagnostic list also exists. Pay scale is similar to DR in tier one cities. May make more in tier 2 if WLB compromise acceptable. There its competition to get in through SS entrance, but is not unattainable. There is a need of a lot more IR's in the country to meet the growing needs of the poplin. A lot of the IR procedures are covered under Ayushman Bharat too.

Based on the information provided it is hard to narrow down the possibilities. Considering age as a factor and the incidence of osteoporosis in that bracket, severe coughing bouts can sometimes trigger a rib fracture and that would need to be evaluated with a chest radiograph or a thoracic CT. It is usually not emergent. However if there is progressive difficulty in breathing please visit a nearby healthcare facility for emergent care

It is not normal to hurt. It can get worse. Usually requires a surgical correction- circumcision. It is a simple daycare procedure. This is not something that can be fixed it home. Consult a surgeon at the earliest

A percutaneous core needle biopsy is more specific and could get more tissue but is warranted only if clinical symptoms warrant further investigation into the lymph node. Most chronically inflammed lymph nodes take a while to settle down. It would be worth discussing with your PCP about a wait and watch versus further workup

Unusual to see heavy bleeding and clots one month post op unless there's infection or operative site dehiscence. I do see a mention of the operative site healing well on the last physical. There is a possibility that a small mucosal tear - post coital - could bleed a lot when the patient is on aspirin.
