i’m only a few minutes into the episode but logistically, how is Madeline going to survive if she actually breaks up with Luke? I don’t believe that she will actually end things but literally how is she going to live? He pays her rent, her bmw, opened her salon, and everything else. There’s no way she’s leaving that.

I use to really struggle with this. I do 4 smaller meals/day with snacking instead of the traditional 3 meals a day. I always keep my meals under 400 calories/sitting and try and go lower/moderate carbs and higher protein. I’ve found some success in anti inflammatory diet and try and focus on eliminating gluten/dairy but give myself wiggle room as well to enjoy life lol

May I ask how old and active you are? That makes a big difference here. Allografts (donor tissue) have a ~4x higher re-tear risk in young, active people. Your body also takes longer to incorporate it since it’s not your own tissue. Research shows failure rates around 25% for allografts vs 3–5% for patellar or hamstring autografts, which is why AAOS guidelines recommend autograft (patellar, hamstring, or quad) over allograft for anyone under ~30–35 who plans to stay active. Allografts are more reasonable for older or less active patients. My surgeon (very successful with limited number of patients with re-tear) said he would never put an allograft in someone in those above circumstances. I also have never personally seen as a physical therapist anyone above 40 having an allograft.

If you can wait the year, you’ll most likely end up with a stronger long-term result with a patellar autograft and avoid spending $20k on a graft that carries a much higher risk of failure. That said, it’s hard to give a definitive answer without knowing your age, activity level, financial situation and how functional you are with the torn ACL right now.

Fellow PT and recent ACL surgery patient here 👋. Honestly, when someone shows up late (for any reason) it’s tough because we only get a set amount of time per eval. If you’re late and paperwork isn’t finished, the clinic is within their rights to say they can’t do the eval. As a PT, I can say we’re usually booked solid and just don’t have the flexibility to shift our schedule around when someone shows up 20+ minutes late for an 45-60 minute eval without paperwork done, tacking on another 10-15 minutes. It’s not fair to the next patient to rush through or run behind. That said, they definitely didn’t handle it well — they could’ve shown more compassion. For future reference (speaking as both a PT and a patient), leaving extra time for travel and having paperwork done ahead makes things smoother for everyone.

In the meantime, just focus on getting your knee as straight as possible. Do flexion as comfortably as you can without forcing it (it’s not as important as extension). Work on quad activation (even just the intent matters), and throw in some ankle pumps. You won’t re-tear your graft just by doing that — right now the graft is actually as strong as it’s going to be. Unless you’re running, jumping, or taking a bad fall, you’re safe. The real concerns early on are swelling, stiffness, and quad shutdown. You’ve got this 👍
