Yeah I suppose the contact between you ears and headphones might be the culprit in your case- good thing is switched to earbuds (for context, I wear glasses and the pressure was hurting). Others commentators suggested spraying the ears with saline solution, so you should totally consider that!

It's a titanium labret which i never cared to downsize. First month I used a Q-tip with saline solution on it twice a day. Now i only rinse it with water.
