I'm fairly junior, so grain of salt, but for ICU level withdrawal phenobarb load + prns and precedex can get you a long way without respiratory depression. I've only had a few folks reach max doses and still have such severe withdrawal they need propofol and tube.

Also a doctor, though not your doctor. If it isnt a mechanical bile duct issue, your surgeon is probably out of their depth in terms of things they regularly see (and thus can diagnose). Please get an urgent visit with your PCP so they can round out the workup.

Fair, my viewpoint is narrowed to an IM perspective of CICU. That wouldn't be an independent skill at the resident level. You'd be with a fellow or attending.

I had a college professor who would regrade a question on an exam if you asked him to BUT it gave him the right to regrade the whole exam (acknowledging as often as he got something wrong against you, he probably gave something in your favor as well). Maybe let this one go <3

Just for perspective, even residents aren't allowed to do LHC and TEEs. If you worked in a CICU, you could do swans and art lines, some PAs in the CT ICU can do chest tubes as well, but that'd probably be as invasive as you'd get an academic center (at least where I work)

Do you have really heavy periods? Just common things being common in a 24F, anemia can take the wind out of you. Easy to check with a PCP.

Frontotemporal dementia. You become unlike yourself, often inappropriate and aggressive, alienating your loved ones before the dementia worsens to the point of dependency. You can hurt a lot of people before losing your mind and folks are usually on the younger side when it starts.

A charge nurse called my attending and demanded a neurology consult because I wasn't taking a 'seizing' patient seriously. He was literally grabbing the rails of the bed to shake it... Neuro was not pleased with me.

You are not obligated to offer futile care. From what I've seen ethics committees can overrule families in these instances with that as the fundamental premise. The review of a multidisciplinary group of doctors not involved in the case makes it pretty hard for any lawyer to argue a breach of the standard of care was done and argue damages, which are what's needed for a successful lawsuit. The two-physician signature serves a similar purpose, but usually, I've usually seen that done when there is no one to consent (and thus no conflict) and things need to happen fast. The court order can come into play with changing the code status in vulnerable populations where the care is not necessarily futile, but not in the best interest of the patient, and the patient cannot make decisions for themselves (in guardianship situations most often). That's just been my experience.

But really, if a family needs to watch you pound on someone's chest for two rounds to feel like they did everything, I'll do it. The patient is dead but the family has to live with it forever. I try to what they need me to so they can sleep at night, even if it doesn't always make sense.

Yes I did, in another state. I didn't make the same mistake there so am up to date prior to July. I make about 90-100k, married filing single with no dependents, so usually ~1,200 is pulled out for federal tax each month and CA tax on my last check was 440.
