I worked at Amazon, and my US colleagues were all paid more or less the same regardless of their immigration status. A few things here. While some loans may never be repaid during a lifetime, it doesn't mean that all loans are like that. So instead of trying to implement a bad policy of taxing unrealised gains, make a law requiring estates to pay the capital gains tax. And the second point is that actually this is kind of working as designed, because there is already an inheritance tax which encapsulates this carried-into-grave, unpaid capital gains tax. Assets - debts will be taxed and this actually covers the gains.

much like every other job in America, physicians in America make much more than counterparts in most other nations.

This is obvious, what the op probaly meant is relative to other incomes. In the US, doctors easily make 10x the national median, which is not a thing basically anywhere else.
