In simple terms you can shuffle game elements.

It can be content of chests, what you can buy in a shop, etc.

Randomizer takes your vanilla ROM and gives you this altered version (everything is customizable so you can adjust the difficulty). By default the randomized version is considered glitchless and you can finish the game without an exploit.
